Care Notebook

I just found a resource on another forum that I thought should be copied and brought to our library of sources. This is something that I could have used months ago, and even considered designing an online version.

I can see how the hard copy version is more useful, as you can carry it to appts. This is a must have for anyone with a medical condition. Don't just think you are doing fine with your scattered info, think of others that might need to pick up the pieces in an emergency. You might know where everything is, but others would be lost. Certainly worth the time and effort to get it organized.

Build Your Own Care Notebook
The Care Notebook has multiple uses. A major role of this notebook is to help parents/caregivers maintain an ongoing record of their child's care, services, providers, and notes. This notebook is a great tool in empowering families to become the experts on their child's care. It is also a way to maintain the lines of communication between the many providers and services that help care for a child and their family
