Ketogenic diet survey

I am the father of a child with severe epilepsy. Eight years ago we decided to go on the Ketogenic diet in an attempt to reduce the seizure activity. This was with great success. Since then I have intensely studied, spoken in public, coached and even lectured on administering the ketogenic diet. Now I am further researching the Ketogenic diet as a treatment for epilepsy by asking people who have administered / used the Ketogenic diet, to fill-in a short survey.
