Hello. I am here here. My child has been diagnosed with Focal onset epilepsy last December. And he has been put on Keppra 500 mg*2/daily since mid of Jan this year when the third seizure occurred. He is 12 years old. And he is a happy boy and healthy otherwise. But yesterday he had one seizure again during GYM class. School called EMS and the ER doctor told us to increase the doze to 750 ng*2/daily. I do hesitate for the increase. His specialist has not got back to me although I called her office three times and sent three emails already.

Anybody has expericne for this? Many thanks. Also, I want to ask, during the 2 and half month mediacation period, he did have four times "one second lip and tongue numbness". And yesterday morning he did have light headache and "electric shot" he described in the early morning. Is this called break through seizure?

Can anybody share your experince and knlowsge with me?

I agree i would definitely want to check with his neuro before upping the meds. A break through seizure is just that, and sometimes it can be caused by lack of sleep, or stress or illness, in which case sometimes they don't up meds (for my daughter if the docs know the reason and it doesn't happen again then we don't necessarily treat it). However if it occurs again or is having more problems then yes you may have to increase his medications.
It is possible that the things you mentioned were auras (aka partial seizures) and were some kind of prelude to his bigger seizure, but it also possible they are unrelated-that is something you should definitely talk with his neuro about.
Did the ER call his neuro when he was there?
 
Yes, they called. We waited another hour and was told an oncall doctor from neurology clinic recommend to up his med to 750. He did not miss any doze. The only thing I can think of is, he went to sleep an hour late the night before. And also he just had two weeks spring break and sleep pattern changed a little bit.( During the break he went bed @ 10:30pm and got up @ 9:00 am) Does the "one second tongue numbness is a peti seizure?

Honestly anything that happened for 1 second i think i would put very little stock in as to whether it was or wasn't a seizure. Sleep deprivation can definitely bring on a seizure.
Generally a petit mal seizure would be considered to be what most call an absence or a "staring" seizure although it's an outdated term and not commonly used anymore since they reclassified the names of seizures. If the numbness were a seizure i would think they would classify it as a partial seizure (which just means it occured in one area (focal), as opposed to a generalized seizure that affects the whole brain).
 
The nurse of the specilaist called me back finally this morning and confrimed that we need up Keppra to 750 mg twice a day. Acutally this morning my son had a two second lip numbness again. So I gave him the extra tablet already before the nurse called me back. I was told, just like you described, those numbness can be aura or nothing to do with the seizure, but more like aura. He also said lift to 1500 mg in total for him is still consider moderate doze. So now he is at 30 mg/ kG level. I pary this is the one. Just keep it. I do not want up the meds anymore. And I hope no side effect for the up.
