I keep a written out seizure diary in a note book but before each appointment I type it into an office style document format that has the month, how many seizures I've had that month, the date and time of the seizure and what I did during the seizure to give to him. I also write down any problems I'm having, questions I want ask and anything else that I think is important that he should know about.

I've asked him a few times if he likes this and he's told me yes but I wasn't sure if he meant it or if he was just being nice. He used to keep the paperwork before I left but now he tells me to give it to the check out nurse when I leave. I didn't know if he was just having her shred it instead of him so at my last appointment I asked the nurse if he liked it and she told me - "He loves it! He has me scan it into the computer to keep with your file because it's much easier to understand than the notes he writes down during your visits! Plus if you're having any problems he will look at the papers you've given him and it helps him figure out things quicker. He wishes everyone did this because most of the time half of a patients office visit is spent just trying to remember how many seizures they've had in one month."

I'm happy to hear that your neuro is pleased with your seizure diary. I know my neuro has told me that it helped him see a pattern in my seizures as to what days of the month and what time of the day or night I was more likely to have any seizures. What really interested them is how they saw more seizure during a low pressure in the weather and how I was having more seizures during the fall and winter, they told me I was having seasonal seizures.
