Opinions on Zonisamide aka Zonegran, please?

Hello again everyone,

I'm the man with the friend who was almost talked into having most of her left temporal lobe removed. With the help of the kind people here, and the Internet, I convinced D not to let them do it. They really didn't have the proper test results to make such surgery a good idea. Monday she went to see her neurologist, and the Doc was kinda cold to D. I expected that, but D didn't. What I didn't like was the main neurologist handed D to another neurologist who may not know much about D's case.

Just to remind all of you, D has had seizures 46 yrs, since she was 14. They have varied over the years, but they seem to be complex partial seizures. Usually she gets 4 - 6 a month, unless she's under extra stress. She takes Tegretol, 800 mg a day. She's been on it about 35 years, and who knows if it does much anymore. In the past it was the only drug that helped at all. Up until 5 yrs ago, D drank at least once a week, but she has totally quit. I think this matters.

So this new Doc wants her to take 100 mg of Zonisamide a day. Zonisamide is the generic form of Zonegran. We both believe in taking Name Brand meds whenever possible, but the insurance co are making this harder and harder.
D is always afraid to take a new drug (I can't blame her), but I want her to try this. She's afraid it will make her too sleepy. So I told her to take it late in the day. I even told her to take 1/2 just to see if it knocks her out. In the past, certain other drugs made her so sleepy she stayed in bed all day. That's a problem because she lives alone.

So I want to hear good and bad reports with Zonisamide. Anything anyone can tell me will help.

I've been starting to have more seizures than usual lately and my neuro thinks it could be due to one of my meds. He said that after taking a medicine for so long your body gets used to the medicine being in it and it stops working for you, if that's the right way to put it. I'm having blood testing done to find out if that's what the problem is right now.

I can't give you any advice on Zonegran because I don't think I've ever been on it. I've always taken generic versions of the medicines and I've never had a problem with those.

When you first start taking a med the side effects will usually be bad. But after a while, maybe a few weeks, then the side effects may ease up. If they don't and you don't like what they are doing and if you can't live with them then it's best to find a new med. I've had to do this before.

I would see my nurse practitioner every so often instead of my actual neruo. The lady was a real b****! I told the office that if I ever saw her again then I was going to change neuros completely. Haven't seen her since. If you're not happy with the neuro that she's seeing then I'd suggest trying to find a new one. Having a dr who will listen to you and understand what you are going through helps a lot.

Neuro said same thing to me about my Keppra dosage. Was told the body developed a tolerance so then I asked does this plateau? Then I got a unsatisfactory answer
 
Hey there, I have been on Zonisamide (200 mg a.m. 300 mg p.m.) since May 2011 as an add on med to my Depakote ER (1000 mg twice daily) to which I have been taking since I was 17. I have been on several meds as add on over the years but nothing ever worked. The side effects with Zonegran that I have experienced were and still do is sometimes not being able to sleep at night so I was given Trazadone plus I have lost a lot of weight which I was happy to do any way. I am thirsty for water alot and it's normal.
Now everyone is different with Epilepsy so it does effect each other differently. Don't forget about that ok?

It's a fact that the same drugs can effect different people differently. Either the Doc or the pharmacist told D that Zonegran may make her more tired. But one difference is you mix it with Depakote and D will be mixing it with Tegratol. The problem with D is she may not take it for weeks. Then she may take it 1 day and not again for a week. She just doesn't want to believe these drugs don't work immediately. This is another reason Doc's give up on her.

Right now a lot of research is being done on epilepsy. Two different hospitals in our area are competing over 2 different surgical methods. They wanted to use D in the research. So long as they thought they could cut part of her brain out they treated her like a queen. Now that she said no, they lost interest in her. I know what will happen if they do the surgery and it doesn't work. Then they will really ignore her. This is why I have to protect her. I always have.
 
I started on 200 mg of Zonisamide a day, along with 600 mg of Lamictal XR. It makes you ridiculously thirsty, and it made me pretty drowsy as well.

Since I started having more activity a few months ago, my neurologist upped the dosage to 500 mg but my body really hasn't responded to it at all. He's actually thinking about taking me off because it really hasn't done much.

I have taken up to 400mg Zonisamide a day and am currently taking 200 mg. At this dose it has been just great. When I was at a higher dose I was very tired. I am not tired and in fact am zipping along [I am yakkidy-yakking here on the forum, for instance.]

When I took 400 mg Zonisamide I was also taking Cymbalta and because of reading another thread on this forum I am now wondering if the combination was the problem. Maybe.

I forgot to mention that at the higher dose the Zonisamide did not entirely eliminate the seizure activity.
 
I didn't have a good reaction to zonisamide. As side effect I lost my appetite, and suffered from related sour stomach, fatigue, and mild depression. It also can be dehydrating (as noted above). Because of the risk of metabolic acidosis, it's important to have your serum bicarbonate tested before going on zonisamide as well as during treatment.
