I wouldn't worry about not having cut down on the Topamax. Fact is, you still have kidney pain a week on, and your GP said to come back in a week if it was still there (i.e. you haven't passed your stone).
That you still have kidney pain is much more important than that you haven't gone off the Topamax. All sorts of damage can be done to the renal system if a stone is present and causing long-term colic.
I don't mean to scare-monger, and I don't know the extent of your pain, but my urologist wrote a scathing letter to a major hospital when they failed to remove my kidney stone, because my kidney was at risk of atrophying (dying).
Doctors have protocols for a reason - your GP wanted you back in a week.
