Does anyone have forms or a webpage for the Mylan Felbatol Patient Assistance Program?

I am having trouble getting my meds. No insurance. I used to get Felbatol through the Meda Patient Assistance Program. However, the manufacturer got sold to “Mylan”. So Meda, the old company, is now Mylan.

I submitted the old forms twice, but apparently the old fax number doesn’t work. I called Mylan twice. The want me to submit a different form. Once they said they would email me the forms, but I didn’t get it. Then they said someone would call me, but they didn’t. I’m running out of meds.

I was not able to find a form online. The old form is still up, but it’s no good. This is the old page: http://www.felbatol.com/patient_assistance.html

Thanks Nakamova, the phone number goes to the main Mylan phone system, where I called before. They weren’t in today. They told me the old fax number is abandoned. You would think that they would forward everything for some time, and accept the old form during transition, but that’s not the case. I get the impression they are still disorganized about buying Meda. I’ll have to insist (nicely) on a resolution when I call tomorrow.

They did seem willing to send it out immediately once they get the form.
