Strange non-seizure behaviour

I'm wondering if anyone has some thoughts on the behaviour of my partner (female 52 yr old) who has recently started on Keppra.

She had her first "event" in April this year - thought she was having a stroke - tunnel vision, tingling in arms, couldn't speak, lost movement in legs. Rushed into A&E where CT scan was clear - to be told this was an aura for a migraine.

Started migraine meds (beta blocker) but had similar events a few weeks apart - always tingling in arms, moving to face, loss of ability to speak or forgetting words, losing vision, intense fatigue.

Just before we got to see a neurologist she had a full seizure - tonic/clonic, frothing at mouth, loss of consciousness. On the back of that she had an MRI scan, which was clear and the neurologist put her on Topiramate.

Three days after starting the topiramate she was in hospital after what sounds like a partial seizure - couldn't speak, very confused, non-responsive, clumsy - and into a full psychotic event. She was paranoid, delusional, didn't sleep for 5 days straight, was convinced the staff were trying to kill her, talking complete gibberish. They stopped the topiramate and over the course of the week she calmed down and back to relative normality. Had another MRI - which was clear; an EEG - which was normal (even though she was still "tripping" at the time and had not slept for 3 days); and a Lumbar Puncture - which was also clear.

After a week at home relatively normal, she had another event of stumbling, followed by inability to articulate clearly. For a week after this she was like a zombie - distracted by the simplest things (you couldn't get her to tear herself from the TV) - could answer simple questions relatively clearly, said she felt fine, but a lot of her speech was completely unrelated to the question - and some questions would take 15 minutes to get a yes/no answer - interspersed with lots of ums and ahs and long silences. She was also very tired in the afternoons and spent a lot of time sleeping. Took her to the docs and back to A&E for a CT scan - which was clear.

Then she had another partial seizure - again looked like a stroke - very agitated, clutching and de-clutching bedclothes with a vice-like grip, moaning but unable to speak at all. Rushed back to A&E - CT scan clear. Kept in for 4 days and diagnosed as having an epileptic partial seizure.

The day after she was clear as a bell in the morning, but by evening was "slowing down" and introducing random comments into her conversation. By the second day she was talking complete nonsense again - couldn't even recoginise her sister.

Docs put her on Keppra and sent her home. Three days later she's still quite confused - yes/no answers (she used to be very alert and chatty) - almost zombie-like at times - very easily distracted. Almost no independent thoughts - but quite calm and happy. Vacant I guess I'd describe it.

We're waiting to get back to the neurologist.

In the meantime, I'm worried that her behaviour is odd - I've read about post-ictal confusion but that seems to be for a few hours - this has been going on for days.

I've heard people talk about some level of confusion/poor memory on Keppra - but this behaviour is similar (although not as bad) as before she started taking it.
 
İ have been on Keppra for a few months and a few weeks ago they increased my dosage to 2000mg a day. I had some of those side effects poor memory and irritability ad well as some confusion such as searching for a word. My symptoms do not seem to be as severe as hers and have abated. I do not like this drug so you might want to contact her doc as soon as possible to discuss this lots of other people have had issues with it.
 
It sounds like she could be having very frequent complex and simple partial seizures. This could be what's causing things.

When I was first diagnosed with epilepsy, 12 years ago when I was 27, I was having at least one a day. Now I'm having on average 5 a month.

During my simple complex seizures I don't black out but I'll have tingling in my fingers and lips. I might get confused about things and feel dizzy but I still pretty much usually know what's going on and I'm able to answer questions ok.

During my complex partials I black out and don't remember I have them. My husband (who sees most of them) says I'll either stare, baby talk which is just gibberish not words at all, or answer questions but never give the right answer. He asked me who he was once and I thought he was his brother. He asked me what the cat was and I told him the cat was another type of animal.

I did have a seizure when I was on the phone with my friend once which I remember being on the phone and talking to her the whole time. She said it was taking me a long time to answer questions and when I did I didn't answer most of them right. I remember answering the questions but I thought I was giving her the right answer. She got worried that I was having a seizure so she stayed on the phone with me and called my husband, who was in the house, on his cell to check on me. Turns out I was having a seizure. I don't know what type of seizure you would call it. A very bad simple or not quite a full fledged partial?

When I first started having them my short term memory was horrible. I'd forget things that had happened that day and even days before. I'd gone shopping with my mom once and had a seizure later in the day I had a seizure. I didn't remember going shopping at all. I might have done something a few days or a week but after a seizure I didn't remember it.

My short term memory is better now, I don't forget things that happened before the seizure, but my long term memory isn't that great. It takes about 3 or 4 months before I start forgetting things.

Not everyone has side effect or the same side effects with meds. Keppra might work fine for her and she doesn't have any problems with it or it might not. I had to go through a few different meds before I found the ones that lowered my seizures and I had no side effects or was able to tolerate them if there were any.
 
I'm assuming that most of the aura symptoms she's been having since April were probably partial seizures rather than straightforward migraines. And that they have now just escalated into worse seizures.

I wasn't given much info with the meds from the hospital - so not really sure now what I'm seeing. Hopefully things will settle down a bit. At the moment, I can't really leave her on her own.
 
I agree with Valeriedl. What you describe your partner is experiencing sounds like frequent CP seizures, maybe even status at times. I've gone status several times, and the ER docs thought I was psychotic, not making sense because the seizures can do that to folks if not under control. Right now I take Keppra- 2000 mgs. a day plus a small dose of Topomax.

Keppra can cause depression/rage in some.
