It's a good idea to have a written/typed list of things to give to drs and any other time that you need it, if you were to go to the ER for example.
On the list I have:
-My name
-Address
-Telephone number
-Date of Birth
-Medical contact (a spouse, parent etc)
-If you wear glasses and/or contacts
-Any allergies that you have (to food or medicine)
-Any types of testing that you cannot have (a MRI for example)
-All the meds that you take, dosages and when you take them (I have this put in spread sheet form, it's easier to read and understand this way)
-A list of surgeries (having wisom teeth removed is a surgery), when they were done, drs name and phone number
-Your primary care physician name and phone number
-Any other drs name and phone number that is important, like your neuro, who you see on a regular basis for something that is wrong
-Your pharmacy and it's address and phone number
-And any other medical information that is important
Keep the list somewhere that is easy to get, like on the fridge, and a copy on you. This way it is readily available and you don't have to search around for it if you are in a rush.
Unfortunately my grandfather had to go to the hospital my ambulance recently. When the EMTs were asking questions about what meds he was on and other things I gave them the list I made for him. I thought the EMT was going to hug me! He said that was wonderful and he wished everyone did that because it makes things so much easier for them.
