It's a good idea to have a written/typed list of things to give to drs and any other time that you need it, if you were to go to the ER for example.

On the list I have:
-My name
-Address
-Telephone number
-Date of Birth
-Medical contact (a spouse, parent etc)
-If you wear glasses and/or contacts
-Any allergies that you have (to food or medicine)
-Any types of testing that you cannot have (a MRI for example)
-All the meds that you take, dosages and when you take them (I have this put in spread sheet form, it's easier to read and understand this way)
-A list of surgeries (having wisom teeth removed is a surgery), when they were done, drs name and phone number
-Your primary care physician name and phone number
-Any other drs name and phone number that is important, like your neuro, who you see on a regular basis for something that is wrong
-Your pharmacy and it's address and phone number
-And any other medical information that is important

Keep the list somewhere that is easy to get, like on the fridge, and a copy on you. This way it is readily available and you don't have to search around for it if you are in a rush.

Unfortunately my grandfather had to go to the hospital my ambulance recently. When the EMTs were asking questions about what meds he was on and other things I gave them the list I made for him. I thought the EMT was going to hug me! He said that was wonderful and he wished everyone did that because it makes things so much easier for them.
 
valeriedl, while I have been keeping a list of my meds, that has been one copy in my wallet... gotten asked so many times by doctors and in hospital, finally I just started my list. But you have a great list to use here with info I can pretty much guarantee I would not be able to answer or share if requested by someone. I will certainly be increasing what Inhave on my list and keep a second copy set up somewhere I would remember just where I have it.

This is what MedicAlert has listed, so I have a card with these things listed that I keep with me and they send me a copy with all these things listed that I do keep on my fridge.
Plus I do were a MedicAlert bracelet.

I keep a list of my meds but it never occurred to me to put contact lenses and so on.

This has given me something to do!

We make military dog tags here at the shop I work at. When people ask what they can have printed, I say, "anything", but this is one of the suggestions I make. It's always good to have the information there and ready to go.
 
military dog tags sounds good but but for some they be very limited as to just what data can go on them... me I use a piece of paper and very small print (for me). In today's world though one could put all the data on the internet and on this dog tag main data and internet link... just the thought in my mind via a great idea for some of us (possibly nearly all)
 
Do you know how they insert these tiny "chips" into our animals now? It has all their medical information, surgeries, owner, address, etc. Well I think eventually it will be happening in humans, just to make it easier for the medical facilities as they become more into this new age of computer technology. Mark my words, one day that's how we will all be tracked. Kinda strange thinking of it though, huh?

Wasn't there a movie or tv show made not too long ago that had something like that in it?
