My understanding is that they leave the wire that connects to the nerve alone because there's more chance of damaging the nerve otherwise.
I know Birdbomb is the resident expert on VNS's.
You might want to check her site & see if anyone has posted anything helpful there.

Hi Junebug.
Has your doctor run an interrogation on your unit? He also needs to check the leads. The generator is normally all that is replaced unless there are problems with the leads. But after 10 years it maybe best to put in new leads too. They do not touch the electrodes at all. I see you've registered at my site so go have a good look around!

Hi Junebug,
I had my first VNS installed in 1997 and it lasted for 5 years and my 2nd one was replaced in 2002. Only the generator was replaced and it was an outpatient surgery. You might want to check out this website for more info: http://us.cyberonics.com/en/vns-therapy/
